District SA-132 (Bonds & Refunding Bonds) and SA-135 (BANS) Borrowing Forms
As a reminder, please submit your completed SA-132 (Bonds and Refunding Bonds) and SA-135 (BANS Borrowing Forms) forms. SA-132 and SA-135 are due by September 1st. Send your district building forms via email to Buildingforms@nysed.gov, including your district’s name & the relevant form submission type (SA-132 or SA -135) in the subject line. These forms are required if the district issued debt between July 1, 2024 and June 30, 2025, including refunding debt and/or EPC entered into.
