District SA-132 (Bonds & Refunding Bonds), SA-135 (BANS) Borrowing Forms and SA-139 (Request for Building Project Data) Building Aid Forms

As a reminder, please submit your completed SA-132 (Bonds and Refunding Bonds), SA-135 (BANS Borrowing Forms) and SA -139 (Request for Building Project Data) forms. SA-132 and SA-135 are due by September 1st and SA-139 are due by September 30th. Send your district building forms via email to Buildingforms@nysed.gov, including your district’s name & the relevant form submission type (SA-132, SA -135 or SA-139) in the subject line. These forms are required if the district issued debt between July 1, 2024 and June 30, 2025, including refunding debt and/or EPC entered into.
