Forms available for online submission are:

  • Hand Sanitizer Use in Classrooms (optional)

  • Student User Agreement

  • Discrimination and Harassment Policy (notice)

  • FERPA (optional)

All other required forms must be filled out and returned to school personnel. Optional forms included in the online forms submission are able to be skipped.

Student Health Form and Emergency Treatment Authorization

Authorization to Administer Medication

(required only if medication administration is needed for your student)

Over-the-Counter Medications

(required only if you’d like your student to receive OTC medication if needed)
