Students with Disabilities

  • Students who have an Individualized Education Plan (IEP) written by the Committee on Special Education, should send a copy to Questar III Home Instruction Services for the files.
  • If you would like your student assessed by the Committee on Special Education, contact your local district or Questar III’s Home Instruction Services by e-mail at homeschool@questar.org

 

Step 1: Complete the letter of Intent and Attendance Requirement Statement

Step 2: Complete the IHIP

Step 3: Quarterly Report at the end of each quarter.

Step 4: An Annual Assessment Report must be submitted with the Fourth Quarter Report.

Step 1:

Each student must attend instruction that is:

  • Substantially equivalent to 180 days of instruction per school year (For grades 1-6 this is 900 hours per year, for grades 7-12 this equates to 990 hours per year)
  • Absences are permitted on the same basis as the District’s attendance policy
  • Records of Attendance must be maintained and made available to the District upon request
  • Instruction provided at a location other than the primary residence of the student must be in a building that is in compliance with the local building code for health and safety purposes.

 

Step 2:

  • The IHIP must list the syllabi, curriculum materials, textbooks or plan of instructions to be used in each of the required subjects
  • The IHIP must include: How many units you will cover this school year based upon; one unit means 6,480 minutes of instruction per year
  • The IHIP must include: the dates in which you intent on submitting your quarterly reports.
  • The IHIP must identify the name(s) of individual(s) providing instruction.
  • The IHIP must include how you will cover the following topics each year K-12 as stated in the NY State Home School regulations:
    1. Patriotism and Citizenship
    2. Health education regarding alcohol, drug and tobacco misuse
    3. Highway safety and traffic regulations, including bicycle safety
    4. Fire and arson prevention and safety
                        1.  

FOR GRADES 1-6, PLEASE PROVIDE INSTRUCTION INFORMATION FOR THE FOLLOWING SUBJECTS:

  • Arithmetic
  • Reading
  • Spelling
  • Writing
  • The English Language
  • Geography
  • United States History
  • Science
  • Health Education
  • Music
  • Visual Arts
  • Physical Education

*You must include instruction in bilingual education and/or English as a second language where a need is indicated.

FOR GRADES 7 and 8 PROVIDE INSTRUCTIONAL INFORMATION FOR THE FOLLOWING SUBJECTS:

  • English (two units- one for grade 7 and one for grade 8)
  • History and Geography (two units – one for grade 7 and one for grade 8)
  • Science (two units – one for grade 7 and one for grade 8)
  • Mathematics (two units – one for grade 7 and one for grade 8)
  • Physical Education (on a regular basis)
  • Health Education (on a regular basis)
  • Art (one-half unit)
  • Music (one-half unit)
  • Practical Arts (on a regular basis)
  • Library Skills (on a regular basis)

The Units required are cumulative requirements for both seventh and eighth grade

FOR GRADES 9-12  PLEASE PROVIDE INSTRUCTIONAL INFORMATION FOR THE FOLLOWING SUBJECTS:

  • English (four units)
  • Social Studies (four units)- One unit of American History, ½ unit of Participation in Government, ½ unit in Economics
  • Mathematics (two units)
  • Science (two units)
  • Art and/or Music (one unit)
  • Health Education (one-half unit)
  • Physical Education (two units)
  • Electives (three units)

The units required are cumulative for ninth through twelfth grades: For example, ELA  one unit each for grades 9,10,11, and 12. 

Step 3:

You must submit Quarterly Progress Reports to Questar III providing the following information:

  • The total number of hours of instruction received during the quarter;
  • A description of the material covered in each subject listed on the IHIP;
  • Either a grade for the student in each subject, or a narrative evaluating the student’s progress; and
  • If less than 80% of the course material set out in the IHIP was covered in any subject, the report must include a written explanation.

Step 4

An Annual Assessment Report must be submitted with the Fourth Quarter Report.
The Annual Assessment must include one of the following:

Commercially published norm-referenced achievement test

The test shall be selected by the parent from one of the following

 

  • The Iowa Test of Basic Skills
  • The California Achievement Test
  • The Stanford Achievement Test
  • The Comprehensive Test of Basic Skills
  • The Metropolitan Achievement Test
  • A State Education Department Test
  • Another test approved by the State Education Department

An Alternative Evaluation approved in advance by the Superintendent

  • Grades 1-3
    A written narrative prepared by:

• A New York State Certified Teacher
• A Home Instruction Peer group review Panel
• Or other person, who has interviewed the child and reviews a portfolio of the child’s work

  • For grades 4-8
    A written narrative can be done once every other year, prepared by a:

• A New York State Certified Teacher
• A Home Instruction Peer group review Panel
• Or other person, who has interviewed the child and reviews a portfolio of the child’s work

Grades 1-3

  1. A written narrative prepared by:
  • A New York State Certified Teacher

  • A Home Instruction Peer group review Panel

  • Or other person, who has interviewed the child and reviews a portfolio of the child’s work

or

Commercially published norm-referenced achievement test

Grades 4-8

Commercially published normreference achievement test

or

(Every other year you may use)
