  • English Language Learners (ELL) Survey 

  • Form FB Building Condition Survey Expenses 

  • Form FB Projected Capital Outlay Exception Aid (COEA) Details 

  • Form FB Water Testing Survey 

  • Form set FT Schedule G ZEB Infrastructure Verification 

  • Form set FT Schedule J Expenditures Verification 

  • SAMS Summer Transportation Expense for Certain Special Education Programs 

  • ST-3 English Language Learners Expenditures vs Students 

  • ST-3 Universal Pre-Kindergarten Expenditures Verification 

  • Students in Equivalent Attendance Programs and Coordinating Hours 

  • Transportation Staff Salaries and Benefits 

Select the Claim Verification menu on the SAMS menu bar, and then click on Claim Verification Project list and view your assigned project name(s). 

If the “District Action Required” column contains a “YES”: 

Your District must provide additional information to the State Aid Office to support your state aid claim in those areas. Click on your required project to view the specific instructions. After you have read the instructions and reviewed the relevant Claim Verification Form(s), click on the “acknowledgment” link at the top of the designated claim verification form. 

If the “District Action Required” column contains a “NO”: 

Your District has not been selected to participate in that project, and no further action is required for that project. However, you may view the Claim Verification Forms for these projects if you would like more detailed information about the projects.

The PTRC must be submitted to SED using SAMS by the end of the next business day following adoption of the report card, but no later than 24 days prior to the Budget Vote.

The threshold for the 2026-27 school year is all administrators with superintendent in their title, regardless of budgeted salary, and any administrator or supervisor with a budgeted salary of $179,000 or more needs to be included.
