SA-132 and SA-135 are Due September 1, 2018
Pursuant to New York State Education Law 3602 Subdivision 6, districts must complete and submit their SA-132 and SA-135 forms for ALL district Bond and BAN borrowings.  Therefore, if your district issued debt for school construction between July 1, 2017 and June 30, 2018 you must submit the SA-132 (for bonds or capital notes) and/or the SA-135 (for BANS) to SED by September 1st.  This information is necessary to calculate the statewide average interest rate used for the assumed amortization of capital construction projects. Copies of these forms can be found at the following links: SA-132 & SA-135Please note, the actual debt service reported on this schedule is not used as a basis for payment, instead it is used to calculate the statewide average interest rate. 

NOTE: SA-132 forms are also required to be submitted for Refunding Bonds and Energy Performance Contracts (EPC).

2018-19 Consolidated Application for ESSA Funded Programs is Due by August 31st
NYSED recently developed an online process to complete the 2018-19 Consolidated Application for ESSA-Funded Programs.  To access this application you may log into SED’s Business Portal by using the following link: https://portal.nysed.gov/abp.  The authorized user will find this application under the ‘SED Monitoring and Vendor Performance System’ link and then the “Title I School and Community Services” tab.

Districts are required to complete all sections of the application and must answer each question marked with a red asterisk.  Please keep in mind, the online application may only be submitted/certified by the chief school officer of the district.  For further instructions on completing this application please visit: http://www.nysed.gov/essa/schools/consolidated-application.

Although districts are no longer required to send hard copies of the Consolidated Application for ESSA Funded Programs to SED, hard copies of the FS-10 Budgets and Budget Narrative forms will continue to be submitted via mail.  School districts must send signed originals and two hard copies of each FS-10 Budget Form to SED using the following address:
