Questar III’s School Social Worker – Family Support service provides intensive case management to the family/guardians and student who may require the following:
- initial referral(s) to agencies for the family/guardian(s) and student, especially kindergarteners with moderate to severe disabilities, and/or
- assistance with developing community contacts and accessing services, and require at least weekly follow-up.
- intensive support to maintain care for their child, such as keeping up with psychiatric, medical, and parenting strategies.
- intensive support during a major transition, i.e., death, illness, change in living arrangement, etc.
